How the referral process works

Start With an Informational Call
A complimentary 15-minute call is available for patients who have general questions or are
unsure which program may be appropriate.
This call is informational and is not a medical evaluation. No diagnosis, treatment decision, or
prescription is provided during the call.

Complete a Medical Evaluation
A Klearmind physician reviews your health history, medications, previous treatments, treatment
goals, and relevant safety considerations.
Completing an evaluation does not guarantee treatment or issuance of a prescription.

Receive an Individualized Treatment Plan
If treatment is considered medically appropriate, the physician explains the recommended
program, anticipated schedule, potential benefits, material risks, alternatives, and follow-up
requirements.

Begin the Appropriate Program
Treatment takes place according to the selected program:
- IV ketamine is administered at the Klearmind clinic.
- Oral KAP dosing sessions take place at the participating therapist’s office.
- At-home oral ketamine sessions take place according to the physician-directed treatment plan.

Complete Follow-Up Care
Response, adverse effects, medications, safety considerations, and the appropriateness of
continued treatment are reviewed during follow-up care.
Treatment schedules may be changed or discontinued based on the physician’s clinical judgment.
Talk with your VA care team
Discuss your condition, prior treatment, and whether a Community Care referral may be appropriate.
VA determines eligibility
VA—not Klearmind—determines Community Care eligibility and whether to authorize the requested service.
VA sends an approved referral
The authorization should identify Klearmind, the approved service, number of visits or units, and valid date range.
Klearmind verifies the authorization
We confirm the referral details before scheduling covered care. Authorization does not guarantee clinical eligibility.
Complete a Klearmind medical evaluation

Is Klearmind a VA Community Care provider?
Yes. Klearmind is an approved VA Community Care provider.
Does VA Community Care automatically cover treatment?
No. Services are covered only when Klearmind receives an approved VA referral or authorization.
Coverage is limited to the services, number of visits, and authorization period identified in the approved referral.
Veterans should confirm authorization details with their VA care team before beginning treatment.
Does Klearmind bill insurance?
No. Klearmind does not bill or submit claims to commercial health-insurance companies. Services are self-pay, except for services specifically authorized through an approved VA Community Care referral. Klearmind can provide a superbill upon request for patients who wish to submit it to their insurance plan for possible out-of-network reimbursement. A superbill is not a guarantee of coverage or payment. Patients are responsible for confirming their benefits and submitting the claim directly to their insurance plan.
